--smush-placeholder-aspect-ratio: 523\/779;\" \/><figcaption id=\"caption-attachment-10566\" class=\"wp-caption-text\">The incision and subsequent scarring depends on the patient&#8217;s choice and on each particular case.<\/figcaption><\/figure>\n<h4><span class=\"s2\">Inframammary route<\/span><\/h4>\n<p class=\"p1\"><span class=\"s1\">This is the most anatomical and surgically logical approach, as it <strong>uses a natural fold to conceal the resulting scar<\/strong>.<\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">It allows access to the surgical plane <strong>without incising the mammary gland or crossing its ducts<\/strong>, which connect the gland to the exterior via the nipple and may be colonised by bacterial microorganisms.<\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">For this reason, this approach:<\/span><\/p>\n<ul>\n<li class=\"p1\"><span class=\"s1\">Keeps the <strong>breast &#8220;clean&#8221;<\/strong> from a radiological standpoint.<\/span><\/li>\n<li class=\"p1\"><span class=\"s1\">Minimises the risk of intraoperative contamination of the implant.<\/span><\/li>\n<li class=\"p1\"><span class=\"s1\"><strong>Preserves the sensitive nerve endings<\/strong> that terminate in the areola.<\/span><\/li>\n<\/ul>\n<h4><span class=\"s2\">Periareolar route<\/span><\/h4>\n<p><span class=\"s1\"><strong>The scar is placed on the periphery<\/strong> of the lower hemicircumference <strong>of the areola.<\/strong><\/span> <span class=\"s1\">This is a very popular route amongst patients because <strong>the resulting scar achieves the desired aesthetic outcome at an early stage<\/strong>, and can become imperceptible when naturally pigmented.<\/span><\/p>\n<p><span class=\"s1\">From the surgeon&#8217;s perspective, this approach, unlike the inframammary scar, <strong>allows intraoperative adjustments<\/strong> to the position of the future sulcus and <strong>facilitates remodelling of the mammary gland<\/strong> in cases where techniques are required to alter shape and distribution, such as tuberous breasts or breast asymmetry.<\/span><\/p>\n<p><span class=\"s1\">It also facilitates management of the lower pole in cases where additional techniques are needed to aid its expansion.<\/span><\/p>\n<p>This approach <strong>requires a minimum areola size<\/strong> and is therefore not recommended for patients with particularly small areolas.<\/p>\n<h4><span class=\"s2\">Axillary route<\/span><\/h4>\n<p><span class=\"s1\">This is a <strong>very popular<\/strong> approach <strong>owing to the advantage of keeping the scar away from the breast<\/strong>, though it is situated in an area that may be more exposed.<\/span><\/p>\n<p><strong>We use this approach at the patient&#8217;s request<\/strong>, limited to cases in which <strong>implants of a moderate size<\/strong> are to be placed that do not require descent of the inframammary fold, in order to achieve an anatomical and optimal result.<\/p>\n<p><span style=\"line-height: 1.5;\">Over our years of experience, we have found that <strong>the scar is one of the aspects that causes the greatest concern<\/strong> in patients prior to surgery.<\/span><\/p>\n<p><span style=\"line-height: 1.5;\">However, once the procedure has been performed, it is something that <strong>fades into the background and does not present any problem.<\/strong><\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">From the first month, we guide patients through the therapeutic options and recommendations available to achieve the best outcome, although it is the passage of time that will give the scar its desired imperceptible and discreet appearance.<\/span><\/p>\n<p class=\"p1\">[vc_empty_space height=&#8221;15px&#8221;]<\/p>\n<h3 class=\"p1\"><span class=\"s1\"><b>Where do we place the implants? By placing the implant beneath the pectoral muscle, <strong>the implant is better protected<\/strong> and more thoroughly covered, which <strong>conceals the presence of the prosthesis<\/strong> and softens the breast contours, achieving <strong>more natural and harmonious results<\/strong>.<\/span><\/p>\n<p><span class=\"s1\">Furthermore, the fact that the implant is covered by thick, highly vascularised tissue provides better protection against possible bacterial contamination, <strong>thereby reducing the likelihood of infection and capsular contracture<\/strong>.<\/span><\/p>\n<h4><span class=\"s1\">Dual plane<\/span><\/h4>\n<p><span class=\"s1\">To be more precise, the technique we use most frequently is the <\/span><span class=\"s2\"><strong>dual plane<\/strong>, a surgical<\/span><span class=\"s1\"> variant of the submuscular plane in which the implant is positioned behind the muscle.<\/span><\/p>\n<p><span class=\"s1\">To achieve this, we section the most distal insertions of the pectoralis muscle at the rib, so that <strong>the muscle does not trap the implant but covers it, leaving it free and in greater contact with the gland in its inferior portion.<\/strong><\/span><\/p>\n<p><span class=\"s1\">The main advantage is that it <strong>prevents upward displacement of the implant<\/strong> and undesirable movement upon contraction of the pectoral muscle \u2014 unwanted phenomena associated with the purely submuscular plane that we have successfully avoided with this technique.
