Mini facelift techniques have been applied for almost a century, with Dr. Passot describing the first surgical technique in 1919. From that time to the present day, mini facelift and neck lift methodologies have been perfected to achieve optimal results through the use of minimal incisions and separations, avoiding incisions behind the ears and on the scalp.

What is a facelift?

Simply put, a facelift is a facial plastic and cosmetic surgery procedure that lifts the deeper tissues of the face and neck back to a more youthful position by removing excess skin.

Types of facelift

Assuming that all surgeons make incisions, the position of the incisions varies according to the surgeon and the procedure, but generally the surgeon will make incisions that follow the contour perfectly in front of the ear, go around the earlobe and then pass behind the ear to the hairline.

The excess skin is then lifted from the SMAS (Superficial Musculoaponeurotic System), which is a fascial layer that wraps around the facial muscles and continues as the platysma muscle in the neck—the one that causes the vertical neck bands.

The fact that the SMAS and platysma are continuous lends weight to my argument that one should not have a facelift without a neck lift and vice versa.

The face and neck were born together, have aged together and should be rejuvenated together to achieve natural facial rejuvenation.

Once the skin is elevated, the facelift surgery really begins. The skin is a passenger during the whole process and should not play any supporting role in the facelift; it simply accompanies the process.

All surgeons do things slightly differently at this stage, but all facelift techniques follow the same pattern.

Imbrication

The SMAS is bundled with a suture loop and fixed in a higher position to the immobile fascia in front of the ear and along the cheekbone.

A strip of SMAS is removed and the edges are sewn vertically, horizontally or obliquely.

Deep plane facelift

In the deep plane facelift, the dissection under the SMAS begins beyond the ears towards the centre of the face, the deep retaining ligaments of the face are released and now unobstructed, the neck, jowls and midface move upward together in a much more intuitive and natural vector.

Preparation for the mini facelift

In the case of mini facelift surgery there are no preoperative medications, but certain supplements and anticoagulants—non-steroidal anti-inflammatory drugs (ibuprofen, Neurofen, Voltarol), aspirin, vitamin E, arnica and alcohol—should be avoided in the week before surgical treatment.

On the day of facial surgery, my patients are registered as day patients.

Once in gown and after confirming their consent, they are escorted to the surgical theatre for the facial procedure by our team of nurses.

The skin and facial tissues are injected with local anaesthetic until completely numb. A small incision is made around the hairline from the sideburns to the tragus cartilage of the ear and the skin is elevated. The SMAS is then imbricated or plicated and the redundant excess skin created is removed and meticulously closed. A facial support garment is placed.

Mini facelift recovery

Patients literally walk out of the operating theatre into the surgery recovery room and change back into their own clothes ready for discharge from the clinic. There is no long downtime due to this facelift procedure.

Overnight at home, the face may feel tight. We recommend the use of ice packs in front of the ears, but nothing that paracetamol alone will not relieve. Occasionally, in the first few hours there may be slight leakage of anaesthetic fluid on the gauze bandage.

During the following days, the face may look puffy, but this starts to improve after the third day. We advise you not to exercise or lean forward after the operation and suggest not to drink too much water or eat salt.

The jaw may feel tense where the muscles have been tightened, but this is normal and passes.

Sutures are removed in our clinic one week after treatment, by which time most of the bruising and swelling of the face has settled.

Most patients can return to work since the recovery time is very short, or even sooner, with their hair down to disguise the signs of facial surgery.

Aftercare

This type of facelift technique tends to heal very quickly as it is minimally invasive.

Cosmetic surgery incisions are short and the extent of dissection is limited.

In very occasional cases a temporary black eye may remain after surgery on one or both sides of the face. The operated area of the face and cheeks may be swollen for a few weeks and the overlying skin may be numb.

However, this usually normalises after a few weeks. The incisions should be kept clean until the sutures are removed, one week after surgery.

How long does a mini facelift last?

It is difficult to establish a timeframe for this facelift procedure, as each face is different.

As the mini facelift is not a definitive repositioning of sagging facial soft tissues with deeper ligament release, the results against the ageing process are more short-lived than with a traditional facelift—that is, a deep plane facelift and neck lift.

With all procedures performed by facial surgeons there are no guarantees, but I have patients with up to 5 years of mini-lifts who continue to enjoy their benefits.

Risks and complications of the mini facelift

No facial cosmetic surgery procedure is without risk; however, these risks are minimised by careful patient selection, adherence to preoperative guidelines and conscientious postoperative care.

The most common complications are listed below, although the list is not intended to be exhaustive.

  • Bleeding: when facial tissues are dissected, haemorrhage of small capillaries creating an expanding blood clot (haematoma) may occur. If treated promptly, this has no impact on the final result other than prolonged bruising.
  • Poor healing: all facial incisions made by surgeons create wounds that heal very thinly and barely visible in almost all patients. Some patients are predisposed to poorer scars that may be raised (hypertrophic), wider (keloid) or white (hypopigmented).
  • Infection: I routinely prescribe prophylactic antibiotics and it is not uncommon to have to use topical antiseptic ointment on facial wounds to keep them clean, but true infection is very rare.
  • Nerve weakness: temporary weakness of the facial muscles that move the lip, mouth or eyebrows may occur after surgery, usually due to stretching, swelling and tissue dissection.
  • Delayed healing process: when the skin is elevated, the blood supply to the skin is temporarily compromised. In patients with already compromised blood flow—smokers, revision cases—skin areas may crust and heal slowly.
